City Connections
City Connections, delivered by Age UK, delivers services to residents and workers in the Square Mile. You will need to be over the age of 18 or be a young carer to use the services.
See below for key services provided by City Connections, or all of their services can be found on their website.
It’s important that all residents can access services that prevent or delay their care needs from becoming more serious.
The below activities are run by City Connections. Please email City Connections or call 0203 957 9844 if you would like more information about any of them.

City Connections can complete a Community Support Assessment with you to determine your needs and what support is available in your local community. For example, there may be a free or cheap exercise class that can help you with physical activity needs.
Please email City Connections or call 0203 957 9844 if you would like more information.
Going into hospital can be a daunting time and it’s easy to feel alone or not sure about your care. Equally, leaving hospital can leave you with questions and worries.
The Community Care Navigator, provided by City Connections, will help you to get the right support at the right time and help you to access that support, all with the aim to ensure you have a safe discharge and reduce chances of hospital readmission in the future.
They do this by putting you at the heart and by linking in with Adult Social Care Social Workers and other Health and Social Care partners.
Please email City Connections or call 0203 957 9844 if you would like more information.
