It’s important that all residents can access services that prevent or delay their care needs from becoming more serious.

Young Carers Activity Club

Every other Monday at 4pm plus trips.

The Young Carers Activity Club is a fortnightly activity that gives young caregivers an escape from their responsibilities as they go on exciting outings. This initiative provides a well-deserved break for young carers, offering them a chance to unwind, connect with peers, and create cherished memories.

City Connections can complete a Community Support Assessment with you to determine your needs and what support is available in your local community. For example, there may be a free or cheap exercise class that can help you with physical activity needs.

Please email City Connections or call 0203 957 9844 if you would like more information.

Going into hospital can be a daunting time and it’s easy to feel alone or not sure about your care. Equally, leaving hospital can leave you with questions and worries.

The Community Care Navigator, provided by City Connections, will help you to get the right support at the right time and help you to access that support, all with the aim to ensure you have a safe discharge and reduce chances of hospital readmission in the future.

They do this by putting you at the heart and by linking in with Adult Social Care Social Workers and other Health and Social Care partners.

Please email City Connections or call 0203 957 9844 if you would like more information.

The Carers connections service offers a wide range of assistance for carers, including:

  • Access to entitlements: Helping carers understand and access their rights and benefits.
  • Information and advice: Providing guidance on a variety of issues that carers may face.
  • Advocacy: Representing and championing the needs and rights of carers.
  • Weekly drop-in sessions: Offering regular, accessible opportunities for carers to engage with the service.
  • Referral services: Acting as a central point for carers to access various forms of support.

The Carers Support Coordinator plays a crucial role in this service by providing one-on-one support, identifying additional entitlements, offering basic welfare rights advice, and guiding carers to more comprehensive community resources.

Additionally, the service organises the City Carers Forum to amplify the voices of carers and shape the service delivery.

This holistic approach ensures that carers receive the support they need and have their voices heard.

For further information or to access support please email Angela Wareham, Carers Connections Coordinator, visit the Carers Connections website or call 07380 182 458.
