You can find out more about dementia on the NHS website and you can see how a GP can help on the Alzheimer's Society website.

There are a range of services that you can use to support your wellbeing and connect with others. For more information, visit the Services, Advice and Guidance for adults page.

The City of London are part of the award-winning City and Hackney Dementia Alliance which was formed to bring together health, social care and voluntary sector organisations to work within a framework of formal partnership. The aim, to achieve greater collaboration and integration. The City of London is fully committed to the alliance and it’s aims to improve the lives of people living with dementia and their carers.

The East London Foundation Trust, as well as being a key member of the alliance, run the City and Hackney Dementia service, a new model of dementia care which provides a diagnostic service that holds patients from diagnosis to end of life, or out of borough placement. It ensures everyone living with dementia has a timely diagnosis, with greater access to ongoing help, advice and support for them, their carers and families.
